How to Register with The Practice and Register for GP Online Access

To register, please complete a GMS1 New Patient Registration Form and a New Patient Questionnaire for Each New Patient, please download from the next page or collect from reception. Please read the Summary Care Record Opt-Out information and print this out if you would like to opt out of the information sharing agreement.

From January 2015, all GP practices in England can register new patients who live outside of their practice boundary area. You are able to register with practices in more convenient locations, such as a practice near your work or closer to your children’s schools. This voluntary arrangement will provide you with greater choice and aims to improve the quality of access to GP services.

If the practice feels it is not clinically appropriate for you to register so far away from home, they can refuse and explain why. You may wish to join a GP near work or re-register with your old GP following a move. The NHS Choices has a website link which compares surgery facilities, services or performance.

The practice will decide when they receive your registration form whether to accept you as a regular patient or accept you without offering home visits.  Because of the greater distance to your home, the GP is under no obligation to offer home visits. If you’re not well enough to go to the practice yourself then other arrangements will be made by calling NHS 111.  

NHS England ensures there is access to a service either near your home or at home. When you register with a practice away from home you’ll be given information about what to do in those circumstances. If you’re too ill to attend the practice in person, or the practice can’t help over the phone, call NHS 111. They can tell you about local services or how to arrange a home visit.

We accept new patients who live in our practice area which has been approved by Cambridgeshire and Peterborough Clinical Commissioning Group (CCG) and is approximately an 8 mile radius of Great Staughton.

When you register we will ask you to make an appointment with a nurse for a new patient health check to ensure our records are accurate and that all routine checks (blood pressure, smear tests, mammograms etc.) are up-to-date. If you are on regular medication you will also need to see a doctor so that your medication can be reviewed and entered on the computer.

Please bring the completed forms along to the surgery with your NHS number and 1 form of Identification. Once you have been accepted as a patient, your medical records will be transferred to us and a new medical card will be sent to your home address. You do not need to inform your existing practice that you are moving although it may be courteous to do so.

GP Online Access

By registering at reception for GP Online Access, regular patient's currently have access to the following facilities:

  • to book, view, amend, cancel and print appointments online
  • to order online, view and print a list of their repeat prescriptions for drugs, medicines or appliances
  • to view online, export or print summary information from their record, relating to medications, allergies, adverse reactions and any other items agreed between the practice and individual patient
