How to register as a new patient

How To Register With The Practice

We accept new patients who live in our practice area which is approximately a 10 mile radius of Great Staughton. You can register by completing a registration form which can be obtained from Reception or downloaded from this page. Please also complete the Patient Questionnaire also on this page.

When you register we will ask you to make an appointment with a nurse for a new patient health check to make sure that our records are accurate and that all routine checks (blood pressure, smear tests, mammograms etc) are up-to-date. If you are on regular medication you will also need to see a doctor so that your medication can be reviewed and entered on the computer.

Please bring the completed forms along to the surgery with your medical card if you have it. The information on the medical card will speed up the process of getting your records from your previous practice. Once you have been accepted as a patient your medical records will be transferred to us and a new medical card will be sent to your home address. You do not need to inform your existing practice that you are moving although it may be courteous to do so.
